Who is this form for?
An adopted person who was born in a place outside the state and who has attained the age of 18 years may apply to the Authority for a tracing service, where he or she is seeking to locate a person in that place for the purpose of
- facilitating contact or
- requesting futher information in relation to his or her birth, early life, care or any other matter or medical information relating to the adopted person or his or her genetic relative.
Can the applicant nominate someone to act on their behalf?
Yes, an applicant can nominate a friend or family member (Authorised Person) to help them to complete this form and liaise with their allocated social worker. However, the applicant will still need to apply in their own name. Follow up correspondence will be addressed to the applicant. Here is what is required:
- The applicant needs to sign or make their mark at the end of this application.
- In a separate letter of consent, give the name of the friend or family member who is helping them. Tell us that they are happy to allow the nominated person to act or be contacted on their behalf.
- Date and sign or make their mark at the end of the letter of consent.
- Contact details for the Authorised Person are to be included in Section
- Identification for the Applicant and Authorised Person will also be required.
